Bile, Conjugated and unconjugated bilirubin, TBIL, T bili
Bilirubin is a bile pigment formed by the breakdown of hemoglobin during both normal and abnormal erythrocyte destruction. Direct (conjugated) bilirubin is that portion of the bilirubin that has been taken up by the liver cells to form bilirubin diglucuronide. Indirect (unconjugated) bilirubin is that portion of the bilirubin that has not been taken up by the liver cells. Total bilirubin is the sum of direct and indirect bilirubin present in the specimen. Method is diazotization. Spectrophotometry may be used in neonates six weeks or younger.

Collect specimen in an SST tube. Centrifuge; aliquot serum into a plastic vial and refrigerate. Protect specimen from light (place in brown plastic bag or aliquot into brown tube). Fractionated Bilirubin (BILIFRAC) instead of Total Bilirubin (TBIL) is recommended for neonates <14 days old.
