Which Patients Benefit?

  • Recent hospitalization
  • Diagnosis or recent onset of disease
  • Conditions that require the aid of another person and/or assistive device

Exacerbation of an existing illness or onset of a new disease process, but not necessarily limited to


  • Pneumonia
  • COPD
  • Fluctuating or unstable vital signs
  • Lung Cancer
  • Frequent Emergency Room visits (three or more over six months) indicate the need for a skilled nurse to educate the patient about the disease process and management

Report And Escalate To Care Teams

  • Report a variation on the biometrics and general condition of the patient to care teams
  • Escalate any new symptoms or concerns of the patients to care team for Care Plan Modification
  • Educate the patients and attendants to manage the condition
