Our Telehealth Solutions for Behavioral Health Program offers personalized supportive counselling to individuals coping with mental illness, stress disorders, behavioral health issue and/or addiction. The program is structured to help and support individuals under stress or diagnosed with a mental illness or related disorder and includes those transitioning back into their community after discharge from a hospital. Our experienced clinical care teams offer hope by providing the tools needed to cope and progressively enrich their lives. Behavioral health counselling does not simply treat a condition but addresses the complete ecosystem that includes an individual’s environment, emotions, physical and behavioral health.

WHO NEEDS THIS: One in every three persons are affected by stress and related mental health issues. These could be a result of work stress, post stressful events in life, after debilitating sickness, interpersonal relations at home. There is no need to continue subsisting. Assessment: Complete comprehensive Behavioral Health needs assessment. We ensure confidentiality and anonymity.

Care Plans: Initiation of a structured care plan that covers the Complete health and helps to resolve it through medications, counselling, group therapy. And identification of supplementary community programs.

Tele - Counselling: Regularly scheduled supportive counselling sessions with flexible program durations and levels of service available. Telecommute to your provider's Office from the comfort of your home.

Help Line: Dedicated case management nurses / Psychologist available daily from 8 am to 5 pm 7 days per week

Triage: 24/7 Triage Nurses on call outside of dedicated case manager coverage hours

Physician Escalation: To alternate mental health professionals available as an optional add-on

PHR: Allows the documentation of encounter in the PHR for Care team reviews.

Telehealth: Secure behavioral health telehealth system.

Patient & Provider Web Portal: Web portal access for physicians, caregivers, family and patients to Personal Health records. Ability to view interactive dashboards and Care plans and notes for which they have the appropriate permissions

Alerts & Action Items: We generate appointment/medication reminders, task / activity reminders, Medication and treatment adherence monitoring and Supplementary health education.

Task and Goals: We build agreed Goals for the patients and then define tasks supplemented by its tracking and monitoring to achieve the desired goals.

E-Consult Workflow: This allows you to define your problem, this can be documented by a nurse and then an appointment fixed with your doctor. All patients can connect with providers for their special needs; these can be a part of the Care teams or additional specialists.
